In this section you will find administration forms that may need to be completed from time to time during your employment.

 

They will need to be downloaded, printed completed and then sent to the relevant department or person, preferably by email. Any forms that can be completed and submitted online here are highlighted.  

NEW STARTER FORM

New starter form to be completed by all new employees

PERSONNEL INFORMATION FORM

Personal information that should be completed whenever you have a change in circumstance (eg change of address)

MEDICAL QUESTIONNAIRE

This has to be completed by all new staff to ensure that any health related issues are taken into consideration during your employment. If health conditions develop during your employment, please ensure that this is resubmitted so that we can assess any special requirements that you may have during your work

WORKING TIME OPT OUT

This has to be completed by all new staff to ensure that any health related issues are taken into consideration during your employment. DEATH IN SERVICE EXPRESSION OF WISH

This has to be completed by all new staff to ensure that any health related issues are taken into consideration during your employment. If health conditions develop during your employment, please ensure that this is resubmitted so that we can assess any special requirements that you may have during your work

WORKING TIME OPT OUT

ABSENCE REQUEST FORM

Absence from work needs to be requested and authorised before being taken in accordance with the Absence Policy. Please complete this form for each absence requested

RETURN TO WORK FORM

When returning to work following an absence other than holiday or other authorised absence, this form needs to be completed. This ensures that you are fit to return to work and is a condition of our insurance

SSP SELF CERTIFICATION

Employee's statement of sickness. This is a form that has to be completed for periods of sickness up to 7 days, in order to be entitled to SSP from your employer. This is a Government form that is completed online and printed. It then needs to be sent to the accounts department or your line manager
